Welcome
to Anatomy of an Error, the first in a series of continuing education
programs about patient safety and quality for healthcare systems. Our
curriculum is designed to provide educational opportunities geared toward
the discovery of innovative solutions for safe, accurate and effective
patient care.
This course walks through a medication error that occurred in Denver and
asks you to think about why this incident occurred. As you read the facts
of the case, you'll be asked to stop, reflect, and offer opinions. Of
course, this part of the course will not be graded. You'll get a chance
to see how others have responded and to read our authors' opinions.
HOW TO USE THIS PROGRAM
Carefully
read through the course and consider the facts presented. At several points
you will be asked to consider what common system failures might have occurred.
Your response here is optional, and of course not graded. When you enter
your responses you'll get a chance to see how other participants have
responded.
